TARGETED CASE MANAGEMENT
Able Health Services Inc. will provide Services to coordinate the support of individuals with developmental disabilities. These services are performed primarily by Coordinators of Community Services (CCS) Our agency will focus on these areas:
- Coordination of services for people with disabilities and providing support in the areas that they choose.
- Being able to assist participants to have the best quality and most appropriate services and supports within available resources.
- CCS will assist participants and their families in implementing participant choice, addressing participant satisfaction, and assuring that participants’ needs and preferences are addressed.
Able Health Services Inc. Targeted Case Management Services will provide participants with the development of person-centered, referrals and related activities, monitoring, and follow-up activities.
Able Health Services Inc.’s Coordinators of Community Services (CCS) will assist participants to gain full access to the full range of Medicaid services, as well as access to any needed generic, medical, social, facilitative, vocational, recreational, housing, financial, counseling, legal, and educational and/or any other support services. Able Health Services Inc’s CCS will provide assigned participants, their family members (if appropriately authorized by the participant), and or her legal or authorized representatives (if applicable) with written and oral information about DDA services and the process of developing a Person-Centered Plan. Our CCS will provide each participant, his or her legal representative and family members, (if appropriately authorized by the participant), and his or her legal or authorized representatives (if applicable) with information about the participant’s rights to determine his or her person-centered planning team. Our CCS will assist the participant and his or her team in facilitating the team meeting and creating a Person-Centered Plan. The participant, or his or her legal representative acting on the participant’s behalf, may invite family members, friends, and anyone else he or she may want to be part of team meetings. Participant is encouraged to include important people in his or her life in the planning process. However, the participant, or his or her legal or authorized representative, also has the right to exclude any individual from participating in the development of his or her Person-Centered Plan with the CCS. Our CCS will assist the participant and the team in coordinating generic resources, natural supports, services available through other programs, Medicaid State Plan services, and waiver services. Our CCS provides case management services, including assisting the participant to connect with this array of services and support and ensure their coordination. The Person-Centered Plan (PCP) is the focal point for coordinating services available under various programs, including this waiver, which meets the participant’s needs, goals, and preferences as identified in the Person-Centered Plan (PCP)
The Person-Centered Plan (PCP) must meet the participant’s needs, preferences, desires, goals, and outcomes in the most appropriate integrated setting. Able Health Services Inc.’s CCS will make will be responsible for the following.
- Determine a participant’s needs, preferences, desires, satisfaction, and the most integrated setting appropriate to meet the participant’s needs.
- Assist the participant through planning in choosing goals and outcomes, the services needed to accomplish these goals and outcomes, and the establishment of realistic time frames for meeting these goals and outcomes.
- Advocate for the participant to assure that the participant’s rights are protected, and the participant’s needs and preferences are considered.
- Monitor and act as a third-party advocate for implementation of the PCP; and
- If requested by the Administration, provide utilization review of services being provided to participants.
A CCS hired by Able Health Services Inc. must be competent in the following.
- Coordinating services and monitoring the provision of services.
- Creating person-centered plans which meet participants’ needs.
- Coordinating planning meetings.
- Assisting participants in gaining access to services and supports.
- Negotiating and resolving conflicts
CCS services may be provided to participants determined eligible for services from the Administration, including participants on the waiting list. In the State residential center, a CCS shall be provided for all participants for the purpose of participating in the development of the written plan of habilitation.
